We are not accepting new patients who are currently registered at another practice within our boundary.
How to register:
Please visit NHS Inform Register and download and print the registration forms. One form per patient must be completed in full.
Please download and print the Pathhead Medical Centre New Patient Questionnaire. Make sure to fully complete this form for each family member.
Take your completed forms for each family member to the reception team at the Practice. Please bring along your valid ID. (See below)
If you cannot print these forms at home:
Please ask the reception team for the registration forms. You can complete these in the practice or take them home. They must be signed by each adult. Please hand back to the receptionist.
What happens next?
Acceptance on to the list is as at the discretion of the GP. We will ask you for proof of identity. Once you have been accepted as a patient, your medical records will be transferred to the Practice. Your medical records may take several weeks to arrive at the Practice. In the rare event of a patient being violent or abusive, the patient will be removed from the practice list. The patient will be informed of the reason for removal in writing and the Health Board will be informed.
Please supply two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse care. Overseas visitors may be asked for additional information to prove entitlement. One form of identity must ideally be photographic such as a Passport, Drivers Licence, Official ID card from Public Services body, Student matriculation card current year)
Registration ID Policy
In accordance with Health Board name recommendations, all new patients should present two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.
One form of identity must ideally be photographic such as:
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
Other documents for proof of residency that are acceptable are:
- Recent utility bill (within last 3 months)
- Council Tax document
- Television Licence
- Payslips (last two months)
- Rent book/agreement (Public Body or Private Landlord)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- Divorce Annulment Papers.
N.B. The above list is not exhaustive and staff are entitled to ask for further proof if required. Please note you will not be registered with the practice if you cannot provide the documents required to prove NHS eligibility.
